CL102 - Case Management and Referral Principles and TechniquesEvery client who enters treatment after a screening and initial assessment is examined through an extensive assessment process that allows clinicians to ascertain each particular client's problems or needs. These problems or needs, are then transferred onto the treatment plan or referred to for creating client goals. This is a collaborative process, therefore once the treatment plan is complete, the clients "sign off" on the treatment plan in agreement. |
